Community Nursing

Community nurses collaborate closely with general practitioners, social workers, hospitals, and other medical professionals to deliver personalized treatment. The program enables patients to have the highest quality of life by assisting them in managing their illness and disability.

Assistance with personal care/Shared Living

Provides support with or supervision of daily living duties in a shared living environment with a focus on increasing each individual's ability to live as autonomously as possible

Supported Independent Living / STA / MTA / SDA / ILO

Unleash your potential by living independently with our SIL Houses, and develop your ability to live autonomously with increased self-esteem, improved life skills, and a sense of empowerment.

Overnight Support/Respite

Respite care is intended to support ongoing care arrangements between participants and those who provide care. STA/respite allows the participant to be supported by someone else while giving their caregiver a short vacation from their regular caring responsibilities.

Social & Community Participation

This support category's resources enable clients to engage in skill-based learning to gain self-reliance in using community support resources, such as peer support, mentoring, and skill development, intended to create volunteer support for skill development in the participant's home or community.

Household Tasks

These support items enable participants to maintain their home environment. This could entail carrying out necessary domestic duties that the participant is unable to perform, such as laundry and meal preparation.

Gardening

Provides professional gardening services that save you time. You don’t have to worry about finding the time to keep your gardens free of weeds or manicured the way they should be including weeding, cultivating, and edging your flowerbeds to make sure your property looks its best.

Allied Health

Delivery of health-related services that are distinct from medicine and nursing. Allied health professionals directly provide services to patients or serve in roles that support healthcare providers.

Support Coordination

Support Coordination aids to empower individuals to take charge of their own lives. not only their services, but also on helping participants create and sustain a strong network of both formal and informal supports. Coaching participants and assisting them in building resilience and capacity within their network are other aspects of support coordination.

Group Activities in a Centre/Day Program

These support items assist participants to access community, social and recreational activities provided in a group setting, either in the community or in a centre.
